Confined space work kills experienced people. That is the uncomfortable pattern in the incident data: the victims are usually not untrained newcomers wandering into a tank, they are competent workers who have entered that same space before without incident and had no reason to expect this entry to be different.
What Makes a Space Dangerous Is Usually Invisible
A confined space is not defined by how tight it is. It is defined by restricted entry and exit, not being designed for continuous occupancy, and the potential for a hazardous atmosphere. That last part is what does the damage. Oxygen deficiency, hydrogen sulphide, methane, and residual solvent vapour give almost no warning that a person can rely on. Hydrogen sulphide in particular deadens the sense of smell at exactly the concentrations that become dangerous, so the warning sign disappears precisely when it matters.
Why Familiarity Is the Real Risk Factor
Conditions inside a space change between entries even when nothing visible has changed outside it. Residue continues to off-gas. A neighbouring line leaks past a valve that was assumed tight. Rain or a wash-down alters what is decomposing at the bottom. The space a crew entered safely last month is not necessarily the space in front of them today, which is why testing is required every time rather than inferred from history.
The Rescuer Problem
A large share of confined space deaths are would-be rescuers. Someone collapses inside, a colleague sees it and goes in, and the atmosphere that took the first person takes the second. This is not a failure of courage or care. It is a completely human response to watching a colleague fall, and it is the single strongest argument for having a rescue plan that does not depend on improvisation in the moment.
What a Real Entry Procedure Requires
- A written permit — issued for that space, that task, that shift, by someone competent to assess it.
- Atmospheric testing before and during — oxygen, flammables and toxics, tested at multiple depths, because gases stratify.
- Isolation and lockout — every line, agitator and energy source positively isolated, not just switched off.
- Continuous ventilation — maintained throughout the work, not just before entry.
- An attendant outside — in constant contact, with no other duties, who never enters.
- A rescue plan that has been rehearsed — with retrieval equipment in place before anyone goes in.
Where Programmes Break Down
Almost never in the written procedure. They break down in familiarity. A space that has been entered fifty times without incident starts to feel routine, testing gets shortened, the attendant gets pulled away to help with something, the permit becomes a signature rather than an assessment. The hazard has not changed at all; only the attention paid to it has.
